Rostering is a process by which patients register with one of our family physicians who is affiliated with the North York Family Health Team.
Patients are rostered by signing an enrolment form. The Ministry of Health is advised to add the patient to the physician's primary care roster.
There are many benefits to being a rostered patient including access to a variety of services offered by the North York Family Health Team (NYFHT). These services include diabetic education, heart healthy and smoking cessation programs, access to social workers, dietitians, chiropodists and mental health resources. Rostering also allows for comprehensive preventive care monitoring and chronic disease management.
Rostered patients commit to receiving their primary health care exclusively from their family physician. This is the responsibility we ask you to take on as a rostered patient.
We strongly believe that receiving your primary care from walk-in clinics and home call doctor services does not provide you with the same quality of care. These resources do not usually send consult notes or results to the family doctor. In contrast, our affiliated clinics provide weeknight and weekend care to our patients and we receive a report of the consultation. Click here for afterhours care information.
