Clinic Policies

During the 2024/25 fall/winter respiratory virus season, Influenza, COVID, RSV and other viruses will be circulating in our community.

To protect our most vulnerable patients, as of November 1, 2024, Don Valley Medical Associates will require all patients to wear a mask when visiting our office.

Don Valley Medical Associates will re-evaluate our masking policy as needed based on respiratory virus activity.

Please inform our office in a timely manner if your contact information has changed.  

Please ensure that your health card is not expired (the red and white health card is being eliminated). 

Confidentiality

• The confidentiality of patient information is of utmost importance to us. Unless required by law, we will not release any details regarding you or your child's health without consent. 
• Our office is fully compliant with the Ontario Government's Personal Health Information Protection Act.
• Our electronic medical records software utilizes the most current security safeguards to protect against the loss of personal information and unauthorized access.

Appointments

• Patients are seen by appointment only.
• Appointments are scheduled via telephone or requested online with Medeo. 
• When scheduling your appointment, please inform our receptionists of the nature of your concern to ensure that an appropriate amount of time is booked.
• Please bring all prescription medications to each appointment.
• Please call to cancel at least 24 hours in advance of the appointment. 
• A fee may be charged for missed appointments and late arrivals.

Medical Residents and Students

• Our office is a University of Toronto Department of Family and Community Medicine teaching practice.  You may be seen by a resident or student with your consent.
